The process of getting a heart transplant has become more complex over time. It requires a team that focuses every day on the many different aspects of heart transplant care.
At the start of your journey, this includes knowing all the details and rules of the transplant process and having the ability to quickly recognize when your health is changing.
After a heart transplant, you will need frequent check-ins to ensure that we can identify and treat issues early – often before you even feel symptoms. A dedicated clinic allows us to provide close monitoring in a coordinated, efficient way.
Our goal is to provide you with the most comprehensive, coordinated and high-quality transplant care possible.
Our Approach
In the pre- and post-transplant clinics, you will be cared for by a dedicated team of clinicians who specialize in both pre- and post-transplant care. This means:
More frequent, consistent follow-up: You will come in for routine appointments and, if needed, we will schedule extra visits to support your health. Regular visits allow the transplant cardiologist to catch changes early and adjust your care quickly. Frequent check-ins help the team immediately identify when your condition is changing and when you might need to move up the transplant list or adjust your treatment.
Quicker Evaluation: For patients not yet on the waitlist, having a dedicated clinic helps you complete testing and appointments more efficiently, so there are fewer delays in getting on the list.
Coordinated testing and labs: The clinic will ensure all your required post-transplant testing is completed on time.
Easy access to your transplant team: The clinic includes transplant cardiologists, nurse practitioners, transplant coordinators, pharmacists and social workers who work together to coordinate your care.
Who We Treat
In the Pre-Transplant Clinic, we see patients who are in the heart transplant evaluation process or are already on the heart transplant waitlist.
In the Post-Transplant Clinic, we care for patients who have had heart transplants within the last 3 years.

Patient Resources
The Adult Heart Transplant & LVAD Support Group is open to pre- and post-heart transplant patients and their families, as well as those who have LVADs or who are considering getting one. The group offers social and emotional support as well as educational information.
