The University of Michigan Health Post-COVID-19 Clinic provides care to adult patients experiencing prolonged post-COVID symptoms. This clinic for adult patients is administered through our Physical Medicine & Rehabilitation team.
Patients who have had a COVID-19 infection – whether mild or severe, whether managed at home or requiring hospitalization – can experience a variety of symptoms for months afterwards, including:
- Fatigue
- Shortness of breath
- Joint and muscle pain
- Weakness
- Dizziness
- Chest pain
- Memory problems
- Anxiety or depression
- Insomnia
- Brain fog
- Spine or back pain
Our Approach
Our consultation Post-Covid Clinic at U-M Health is time-limited. Consistent with the emerging science, our care model centers around a six session Post-Covid Recovery Group. These groups are led by our team of neuropsychological and rehabilitation psychology providers and equip patients with education, skills, and interventions to address their current symptoms and optimize quality of life. Over the course of 6 weeks (1 hour per week), participants will gain knowledge and awareness of factors that contribute to their post-COVID symptoms, from the evidence-based bio-psycho-social perspective. Patients will then be provided with clinical interventions and tools with the goal of learning and practicing behavior change, experiencing symptom reduction, and re-engagement in life.
Most patients are also seen by our Advanced Practice Practitioner (Physician Assistant) for assistance with managing their symptoms. Some, but not all, patients will be scheduled with a physician. This program is designed to provide care in addition to care you receive from your primary care team or referring provider. We will be recommending self-management strategies, further evaluation, and/or medication options.
Depending on your symptoms, our recommendations may include physical, cognitive, and/or psychological treatment plans. This clinic does not replace the care of a specialist or a primary care physician for management of ongoing symptoms. Referrals to specialists, orders for labs, and disability paperwork completion are out of scope for this clinic. Patients are seen for up to three visits with a provider, aside from the group, and will be discharged from our care to follow up with their PCP for ongoing management with our recommendations.
Appointment Information
At this time, a referral is required from your primary care physician or inpatient provider to make an appointment. This ensures we receive all of the required documentation necessary to schedule a clinic visit.

Referral Information
Patients must meet several criteria:
- Age 18 or older
- A positive COVID-19 test at least 3 months prior to the referral (PCR/lab test, or home antigen test)
- Continued symptoms post acute COVID-19 infection
- Referral from a primary care provider (or inpatient provider at discharge)
How to refer a patient
In order for us to provide the best possible patient care, expedite the referral process, and schedule an appointment for your patient, we need your assistance. Please fax or send electronically the information listed below to our clinic.
- Completed Post-Covid New Patient Consultation Request form
- Note from last referring provider visit and/or discharge summary from COVID-19 hospitalization
- Reports of any radiological studies performed during or after COVID-19 infection
- Reports of any lab studies performed during or after COVID-19 infection – o Refer to the PASC guide for recommendations on lab tests to order for an initial workup prior to referring to our clinic.
*Please only send relevant documentation pertaining to this referral. If you are including more than 10 pages of medical records, please indicate the page numbers for where the required information can be found so that we can verify that the referral is complete.
Patient Resources
This is an online resource, to support patients and PCPs working together to manage post COVID-19 symptom burden, also known as PASC (Post-Acute sequelae of COVID-19). The purpose of the guide is working together for continued management of symptoms and monitoring/tracking your improvement.
Within the PASC Guide you will find information about specific symptoms, self-care recommendations and interventions for treating those symptoms and self-management tools for tracking process and symptom improvement. These may be helpful to share with your primary care provider during the duration of your post COVID-19 symptom experience.

This treatment guide includes specific symptoms, work-up recommendations, PCP treatment interventions and recommended referrals for further evaluation.
Our Post-COVID Clinic within Michigan Medicine is not currently directly participating with any clinical trials or research studies at this time. We encourage patients to pursue these opportunities independently through the following websites:
