The University of Michigan Health C.S. Mott Children’s Hospital Partners for Children - Little Heroes League program is a community-based pediatric palliative and complex care service specifically designed for children with complex medical needs from birth to age 21 who meet special enrollment criteria.
Our children with medical complexity include those who have medical and or behavioral conditions that impact three or more body systems, have high needs for healthcare services, require the assistance of medical technology and are followed by many specialty clinics. Our team supports these families by guiding them through their child’s journey while navigating the healthcare system and transitions from hospital to home, as well as equipping them with the necessary tools and skills to manage their child’s daily care effectively.
Care for the neonatal population was established and continues to be sponsored by the national Little Heroes League, founded by Kim and Will Ulaszek. Having these partners in the hospital and community leads to more efficient communication, a better understanding of a family’s unique situation and a unified approach to your child’s well-being and quality of life.
Our Approach
Your family will be supported by a team who will build relationships with your child, family, primary care provider and specialists. Every child’s care is designed to meet their unique and complex needs.
Our nurse clinical care coordinators assist with medical advice, clarify day-to-day treatment plans and triage calls. Your primary nurse practitioner will serve as a team coordinator, assist with pain and comfort management and support decision-making. You will also collaborate with social workers who will identify community resources, provide help acquiring supplies and equipment, support decision-making and provide emotional care resources.
Our board-certified palliative care physicians will be part of your team and support your care team throughout your journey. Our child life professionals use many skills to help build relationships to determine specific needs for each child and family member, prepare children for upcoming procedures and assist them in working through feelings about past, present and future experiences. Our clinic lead provides front-line support and appointment coordination.
Appointment Information
Locations of visits include home- or clinic-based appointments (virtual or in-person) every 1-3 months and hospital visits while inpatient.
A primary care provider, specialist or a NICU team member provides a recommendation for eligible patients to enroll in Partners for Children-Little Heroes League. If your child qualifies, a team member will contact you and provide further details on the program and services provided.

Services
The programs supports eligible patients by providing care coordination and palliative care regardless of your child’s location, at home, in the clinics or in the hospital by:
- Providing at-home and virtual appointments every 1-3 months to ease the burden of travel and address concerns or care coordination
- Collaborating with your child’s primary care provider and specialists to manage psychological, social, and physical symptoms
- Preventing, shortening, or helping coordinate hospital stays and discharges
- Providing symptom management and advanced care planning
- Helping with decision making and providing emotional support
- Identifying goals that support your child’s quality of life
- Reducing emergency room visits and facilitating coordination of care with your providers when an ER visit is necessary
- Assisting with important specialty clinic visits and coordinating team communication
- Helping develop a daily and/or emergency care plan
- Facilitating conversations with school personnel
- Connecting your family to available community resources and state programs
Partners for Children-Little Heroes League provides around-the-clock phone support for existing patients with urgent medical questions.
