For hysterectomy and/or vaginectomy (vagina removal) we do not require letters of mental health support by policy. However, most insurance companies require two letters of mental health support before they will authorize insurance coverage for hysterectomies and vaginectomies for gender-affirming care. We strongly encourage patients in need of these surgeries to discuss coverage with their insurance company and initiate the process of obtaining two letters of mental health support for these surgeries. Contact us if you need a referral to a mental health provide for a letter of support.
When speaking with your insurance company to determine whether letters are required for coverage, you may be asked for the "procedure codes" for your surgery. The procedure code (CPT) code for hysterectomy (with or without removal of the ovaries) is 58571; evaluation of the bladder is also performed after all hysterectomies (called cystoscopy) and has the procedure code 52000. The procedure code for vaginectomy is 57110.
You may also be asked for the diagnosis code for surgery. The diagnosis code for gender-affirming hysterectomies and vaginectomies is "gender dysphoria" which has the code: F64.1.
Letters of mental health support for hysterectomy and/or vaginectomy can be sent directly to your surgeon’s office. Letters for Drs. Crissman, Stroumsa, Randolph, Swenson, and Fairchild can be faxed directly to their offices at 734-615-4270.
Therapists may find this template letter useful in drafting a letter of mental health support for you for your hysterectomy and/or vaginectomy. If your therapist does not have experience writing letters of support for medical intervention for transgender people, please encourage them to contact us before writing the letter.
