We may request copies of your recent labs and additional testing may also be ordered to determine the severity of your condition. Our nurses will screen the preliminary information you provide for what will be required. We may also need additional records from your referring physician.

Do I need a referral?
Depending on your insurance coverage, you may need a referral from your primary care physician. Medicare and traditional Blue Cross/Blue Shield patients will not, but most others will.
Please contact your referring physician to obtain that authorization if your carrier requires it. The authorization can be faxed to (734) 936-5830.
