Michigan Medicine HouseCalls provides care to patients that are homebound or have limited mobility.
Our highly skilled medical director, nurse practitioners, care managers and pharmacist work closely with the patient’s primary care provider to help patients stay healthy at home and minimize unnecessary calls to their primary care office or utilization of the emergency department and hospital.

- Care Management
The HouseCalls team services patients that live in Washtenaw County and Belleville, and operates from 8-5pm Monday-Friday. After office hours and on weekends, the HouseCalls medical director responds to the patient. Patients must be referred to the HouseCalls program by a Michigan Medicine provider.
