Your Appointment:
In order to make your visit as effective as possible, we ask that you complete the patient forms and return them at least 72 hours prior to your appointment. Failure to do so could result in your appointment being delayed or rescheduled.
If you need another copy of patient paperwork, call our office at (231) 739-9492.
For your appointment we ask that you:
- Complete and return our forms 72 hours prior to your visit.
- Bring your insurance card & photo ID.
- Bring recent X-rays.
- Be prepared to give a urine specimen.
- Arrive 15 minutes early.
If you cannot keep your appointment, we request 24-hour advance notice to cancel. This makes it possible to give the appointment to another patient.

Additional Information
Payment: Charges for medical services at our office are due and payable at the time services are rendered. Our physicians do participate with Medicare. If you have Medicare, you or your insurance will be responsible for the 20% balance. HMO patients should be prepared to pay your co-pay.
